MRI’s, CT’s AND/OR X-RAYS
Please bring your CD(s) and/or film(s) to your appointment. These are the actual pictures of your radiology test(s) which we need for your evaluation. You may also bring the written MRI, CT or X-RAY reports.
ALL RELEVANT TESTS AND REPORTS
These include any of the following:
- Injection Reports from Pain Management
- EMG & Nerve Conduction Study Reports
- Myelogram Reports
- Referring Physician Notes
